When Should a Wisdom Tooth Be Removed and When Can It Be Monitored?
Not every wisdom tooth needs immediate removal. If a wisdom tooth has erupted normally, is healthy, functional and can be kept clean, monitoring may sometimes be appropriate. The decision depends on the tooth’s position, surrounding tissues, symptoms and long-term risk.

Removal may be considered when a wisdom tooth causes repeated pain or swelling, infection, food trapping, decay, gum problems, damage to the neighbouring tooth or other complications. An impacted or partially erupted tooth may also require closer evaluation because its position can make cleaning difficult.

Importantly, pain is not the only factor. Some wisdom teeth may create problems even before significant symptoms appear, while others may remain healthy without requiring removal.

Clinical examination and appropriate imaging can help assess the tooth’s position, roots, surrounding bone and relationship with nearby structures.

If you have been advised to remove a wisdom tooth, ask not only “Should it be removed?” but also “What findings make removal appropriate in my case?”

The right decision is based on the individual tooth—not simply on the fact that it is a wisdom tooth.
